Instructions: Click to open or click and hold on the links below to save the PDF files to your hard drive. After you have installed Acrobat Reader, open the files and print them from your computer. Fill them out by hand or typewriter, sign, and return to Miracles in Motion.

After completing these three forms and becoming familiar with the Center Rules, volunteers must also read and understand the Miracles in Motion Volunteer Guidelines. Members of the Horse Committee must also read the Horse Committee manual. For each set of PDF files, A PASSWORD IS REQUIRED. PLEASE CALL MIM AT 857-4141 for the password.
