Students must complete new forms each year in order to participate in the riding program. New students need to complete the Application Evaluation first, then have an evaluation at the riding center. Returning students may skip the Application Evaluation unless they have been asked to complete it.

All required forms must be RECEIVED before the student can get on a horse. If, for any reason, you have less than one week in which to get the forms to the farm, you should hand deliver them.
For your convenience, there are two types of forms: Microsoft Word docs optimized for the PC and PDF files, which are viewable and printable from PC or Macintosh computers. To use the PDF files, you must first download and install Adobe Acrobat Reader, a free program available by clicking on this graphic and following their instructions:
The Word documents are best viewed with Internet Explorer and Microsoft Word. With other browsers, you may need to right click on the form name, chose "save Target as" and save it to your hard drive, then open Microsoft Word, and open the form you just saved. You will then be able to print the form.
