TO ALL RETURNING STUDENTS:
Again this year, we are scheduling classes on the basis of age, ability, experience, and need for assistance, as space is available. Please read all of the following guidelines, then follow the instructions at the bottom of the page to proceed to the forms you are required to complete.

To register for class, you must contact the Program Director at:

We will call or send you an email acknowledgement stating your available class time.
Please remember that classes are filled on a first come, first served basis, so please call or email soon!

Forms:

You can access student registration forms via this website. Please download the forms, fill them out and mail them to Miracles in Motion, P.O. Box 14, Cedar Rapids, IA 52406. Do not E-mail the forms. (If you do not have access to a printer, please notify us. We will gladly mail you the forms.)

• Required Forms: Medical Form, Registration and Release, Emergency Treatment, Enrollment Donation Form & Fundraising Participation Forms.
• Additional Forms: Parent Survey, Education Evaluation (if you are currently in school), PT/OT Evaluation (if you are currently seeing a PT or OT, please complete this form.)
• Please review and print a copy of the Miracles in Motion Riding Center Safety Guidelines and Rules to keep for reference.
• All required forms must be in our office two weeks prior to the beginning of classes.
• Please do not return any incomplete forms.

Reminders:

• Our weight limit policy is as follows:
  • Weight limit of 200 lbs.
  • Instructors have the discretion for students weighing between 150 and 200 lbs.
• Any student can be pulled from classes if the Instructor has concerns regarding: safety of the student, safety of volunteers, postural insecurity of rider, and/or welfare of the horse.
• Individual Lessons are available at a cost of $35/lesson; contact the Program Director if you are interested. .
• Approximately two weeks before classes start, you will receive a postcard or an email confirming your registration, class time and date, and the status of your forms.
